Private Duty/Home Care |

We recognize that in many cases specialized home health care, whether for 4 hours or on an extended basis, is often the better choice for those needing assistance. We provide an alternative when institutional care is not practical, economical or appropriate. As most experts agree, the warm familiar and comfortable surroundings of one’s home can be very beneficial as we recover or manage our illnesses. It promotes our health with the feeling of control, independence, reduced stress, and heightened self-esteem. The comfort and control afforded by one’s home can seldom be matched elsewhere.
Our programs are designed to take care of your elderly, ill or physically challenged family member, when you’re at work, away on vacation, or even attending a concert. We will even relieve family members who themselves need time away from caring for their loved ones. What ever your care needs, we have the staff, experience and flexibility to accommodate you. |

How Does it Work? |

During an initial complimentary Assessment/Interview, a registered nurse will visit your home to discuss and develop a personalized care package that will address your health care needs, and can be easily incorporated into your lifestyle. We will:
- Discuss issues pertaining to the client’s physical and emotional needs
- Identify any special care or equipment requirements
- Establish the level of care and discuss the duties and responsibilities of the attending caregiver.
- Clarify your service expectations and ours.
All discussions will be kept in completely privacy and with total confidentiality. Caregivers are placed according to the requirements of the client and in consideration of their experience and proficiency. We attempt to match the caregiver with the client as closely as possible. For example, language may sometimes be an issue, or perhaps a younger patient may prefer a younger caregiver with whom she/he can more comfortably relate. We live in a multicultural community and as such endeavor to accommodate all religious, cultural and ethnic considerations. Certainly every case is inherently unique and must be managed as such. Since our care package is completely personalized, there is significant flexibility in the schedule and provision of care. Our field supervisors will communicate on an on-going basis with appropriate family members and confer with family physicians to ensure that an effective dignified support system is in place. |

Assessing Your Needs |

The creation of a Personal Care Plan begins with an assessment of the type, extent, timeframe, and priorities of the health care services to be provided. Administered in your home by a Registered Nurse, often involving medical practitioners and others, we examine the client’s physical health including illnesses and impairments, mental and social well-being, and any environmental issues, which may affect overall health.
