- Difficulty with grooming, bathing and personal hygiene.
- Difficulty preparing meals for oneself, not eating properly or forgetting to eat.
- Lack of proper nutrition leading to weight loss or dehydration.
- Unable to perform simple household chores such as laundry or vacuuming.
- Administering medications, confusion about dosages or double dosing.
Major Changes in Lifestyle - Diagnosed with life threatening, altering or chronic illness
- Post-surgical care after being discharged from health care institutions.
- Rehabilitation because of impairments such as strokes or the lost of a limb.
- Recovery after vehicle, industrial or other major injuries.
- Recent lost of a spouse, partner or longtime caregiver
- Family caregivers are overwhelmed by care needs or are unable to provide the type and amount of care necessary.
- Care requirements becoming complicated requiring a professional caregiver.
Observed Attitude or Behavioral changes - Sadness, loneliness, isolation, or confusion
- Appearance has changed/deteriorated
- Mood swings, anger, anxiety or depression
- Wandering or easily getting lost
- Difficultly understanding others/remembering names or situations and locations
- Mobility issues such as unsteadiness, increasingly limited movements such as accessing the telephone or doorbell.
- Creating hazards by forgetting to turn off appliances, or allowing tub to overflow.
- Impaired decision making leading to vulnerable situations
